Dr. Robert Gellin DMD, MHS

Dr. Gellin received his D.M.D. from the University of Kentucky, College of Dentistry in 1980. He spent three years in the U.S. Public Health Service, including a one year general practice residency. In 1985, Dr. Gellin earned his Certificate in Periodontics and Master in Health Sciences from the Medical University of South Carolina. Dr. Gellin joined the faculty of the Medical University of South Carolina in 1985 and he is currently Professor and Chair of the Department of Stomatology, Director of the Division of Periodontics and a Diplomat of the American Board of Periodontology. He maintains an active practice limited to Periodontics within the College's Dental Faculty Practice. He has given numerous presentations to dentists, dental hygienists and dental assistants for local, state, national and international dental organizations. Dr. Gellin is also the administrator of the College of Dental Medicine's clinic management software.

Dr. Richard Nagelberg, DDS
Dr. Richard Nagelberg has been practicing general dentistry in suburban Philadelphia for over 27 years. He has international practice experience, having provided dental services in Thailand, Cambodia, and Canada. Richard has served on many boards and advisory panels. He is co-founder of PerioFrogz, an information services company, and is a speaker, clinical consultant and key opinion leader for several dental companies and organizations. He is also a recipient of Dentistry Today’s Top Clinicians in CE, 2009. A respected member of the dental community, Richard lectures extensively around the country on a variety of topics centered on understanding the impact dental professionals have, beyond the oral cavity.
Dr. Robert Cowie
Dr. Cowie is a 1976 graduate of Northwestern University Dental School and practices in Jacksonville, Florida. He holds membership throughout the tri-partite system of the ADA, as well as in numerous professional organizations including the AGD, the American Equilibration Society, the American and Florida Academies of Dental Practice Administration, the Pierre Fauchard Academy, the American College of Dentists and Toastmaster’s International. He has served organized dentistry at the state and local levels, as well as having been a past president of the Jacksonville Dental Society. He has completed over 2400 hours of continuing education, including the continuums at the Dawson Center for Advanced Dental Studies, the Society of Occlusal Studies, and the L. D. Pankey Institute. Dr. Cowie has also served as a visiting lecturer at Northwestern University Dental School and a clinical instructor at the University of Florida College of Dentistry. Dr. Cowie has presented over 200 programs both nationally and internationally while continuing a full-time practice. He was featured in the December 2008 issue of Dentistry Today for the sixth consecutive year as one of the leading educators in dentistry and has published articles in Dentistry Today, Journal of Dental Technology, Journal of the Ontario Dental Association, Contemporary Dental Assisting, and Oral Health.

Dr. Don Deems
Dr. Don Deems is one of only a few dentists in the United States who is a trained professional personal and business coach and practicing dentist. He has the ability to speak to the specific needs and challenges of dentists—because he is one.
Dr. Deems has been in practice for over 21 years, and has received many awards and honors in dentistry, including Fellowship and Diplomat status in several dental organizations. He holds memberships in many others, and is an honor graduate of Baylor College of Dentistry. Dr. Lorne Lavine
Dr. Lorne Lavine, founder and president of Dental Technology Consultants, has over 23 years invested in the dental and dental technology fields. A graduate of USC, he earned his D.M.D. from Boston University and completed his residency at the Eastman Dental Center in Rochester, NY. He received his specialty training at the University of Washington and went into private practice in Vermont until moving to California in 2002 to establish DTC, a company which focuses on the specialized technological needs of the dental community.
Dr. Lavine has vast experience with dental technology systems. He is a CompTia Certified A+ Computer Repair Technician, CompTia Network+ certified and will soon be a Microsoft Certified Systems Administrator. As a consultant and integrator, he has extensive hands-on experience with most practice management software, image management software, digital cameras, intraoral cameras, computers, networks, and digital radiography systems.
Dr. Lavine writes for many well-known industry publications and lectures across the country. He was the regular Technology columnist for Dental Economics Magazine. His articles have appeared in Dentistry Today, Dental Economics, Dental Equipment and Materials, Dental Practice Report, New Dentist, Dental Angle Online, and DentalTown magazine, where is a moderator of ten of their Computer and Software Forums. He has lectured to the Yankee Dental Congress, American Academy of Periodontology, American Academy of Endodontics, the DentalTown Extravaganza, and numerous State Dental Society and Study Club lectures. In addition, he is a member of the Speaking and Consulting Network. He is also the former technology consultant for the Indian Health Service.
Dr. Larry Sangrik
Dr. Sangrik is a 1979 graduate of the Ohio State University College of Dentistry. He completed the intravenous sedation program at Miami Valley Hospital of Dayton, Ohio and the comprehensive externship at Midwest Implant Institute. Having volunteered on the Chardon Fire/Rescue Department, he has personally managed most medical crises, from childbirth to open heart massage.
Active in organized dentistry, Dr. Sangrik has long held numerous positions at the local and state levels. He also has served as a consultant to the Ohio State Dental Board on a task force concerning the use of oral sedation.
Since 1999 Dr. Sangrik is well-known dental CE lecturer. He has been a featured speaker at the American Dental Association Annual Session, the Hinman Dental Meeting of Atlanta, the Yankee Dental Congress, the Chicago Midwinter Meeting, the Rocky Mountain Dental Convention and the dental schools of the University of Mississippi and Creighton University. Additionally, he had addressed numerous state or regional dental conventions. He also has written for Dentistry Today on the topic of dental fear.
Dr. Sangrik maintains a full-time general practice in his hometown of Chardon, Ohio with an emphasis on the treatment of apprehensive patients with IV sedation.
Dr. Daniel E. Becker
Dr Becker received a BS in Zoology at Ohio University and his DDS from Ohio State University in 1972. He is currently Professor of Allied Health Sciences at Sinclair Community College and Associate Director of Education for the General Practice Dental Residency at Miami Valley Hospital, in Dayton. In addition to his academic responsibilities, Dr. Becker provides IV sedation services for dental practices
throughout the Dayton-Cincinnati area. Dr. Becker has published numerous articles in scientific journals, authored chapters in several textbooks and is Coeditor of Management of Pain & Anxiety in the Dental Office. W.B. Saunders / Elsevier
Science Pub, St. Louis, 2002. He has lectured for organizations, colleges and universities throughout North America on topics related to pharmacology, conscious sedation, medical assessment and medical emergencies. Dr. Becker is noted for his
pragmatism and his ability to convert otherwise "dry" material into formats that are both entertaining and informative.
Mrs. Susan McMillan
Susan Weston McMillan graduated with High Honors/Phi Beta Kappa from the University of Florida with a Bachelor of Science in Psychology. She attained her Masters in Counseling Psychology from Nova University with a 4.0 GPA. She is a Licensed Mental Health Counselor and a Certified Addictions Professional.
In 1992 she began a private practice in Bradenton, Florida with a satellite office in St. Petersburg in operation since 1991. She specializes in domestic violence, substance abuse, adult children issues and couples counseling. In 1994, she established the first Duluth Model batterer’s intervention program in the Twelfth Judicial Circuit. Two years later, that program became the very first Certified Batterer’s Intervention Program in the state of Florida. Susan McMillan was a finalist for the Governor’s Peace at Home Award in 1997, and received recognition from Governor Lawton Chiles for her work. Her batterers program has been utilized by the Florida Department of Corrections as a model program for training purposes.
Susan McMillan is a certified continuing education provider through the Florida Department of Health in the area of Laws and Rules as they pertain to the practice of psychotherapy, including confidentiality standards, substance abuse services, abuse reporting and mental health services. She is also certified as a continuing education provider for all Florida Healthcare Providers in the area of Domestic Violence and Medical Errors.

Dr. David Clark
Dr Clark founded the Academy of Microscope Enhanced Dentistry, which is an international academy formed in 2002 to advance the art and science of Microdentistry, Micro-Endodontics, micro-periodontics and dental microsurgery. He is a course director at the Newport Coast Oral Facial Institute. He is on the editorial board for several journals and has been asked to be a contributing author for Endodontic and restorative textbooks. Dr. Clark is the director of the Opinion Leaders Forum “Modern Restorative Dentistry” and serves on several other opinion leaders’ forums for both endodontics and restorative dentistry. He serves on the board of directors for Clinician’s Report (formerly CRA)
Dr. Clark served Clinical Research Associates in the “Update Series” lectures and as an interim Dentist/Researcher from 2005-2007
He has conducted retrospective studies on retention and wear of minimally invasive posterior composites (CRA) and cross sectional studies of coronal fracturing of previously restored teeth. Current studies include strength analysis and marginal leakage of the injection molding and snow plow techniques for class II composites.
Dr. Clark has authored more than 20 articles including the first comprehensive guide to enamel and dentinal cracks based on 16 power magnification in the Journal of Esthetic and Restorative Dentistry. He has written a diverse range of articles relating to minimally invasive dentistry, biomimetic endodontic shaping, and the role of advanced magnification in modern dental practice. Dr. Clark is currently coauthoring the chapter, Contemporary Molar Endodontic Access in the textbook Dental Clinics of North America.
Dr. Louis Malcmacher
Dr. Louis Malcmacher maintains a general and cosmetic private practice in Cleveland, Ohio. He is an internationally known lecturer, and author, known for his comprehensive and entertaining style. Dr. Malcmacher is an evaluator for Clinicians Report, a master of the Academy of General Dentistry, a fellow of the International Association of Dental Facial Esthetics, a fellow of the World Clinical Laser Institute, a contributing editor for Dentistry Today, a spokesperson for the Academy of General Dentistry, President of the American Academy of Facial Esthetics, a monthly columnist and contributing editor for Dental Economics, Dental Tribune, The Dentist’s Network Newsletter, and has been named one of the Leaders in Continuing Education by Dentistry Today for the tenth straight year. Dr. Fred Margolis
Dr. Fred Margolis received his B.S. and D.D.S. from The Ohio State University and his certificate in pediatric dentistry from the University of Illinois College of Dentistry. Dr. Margolis is a Clinical Instructor at Loyola University’s Oral Health Center and an Adjunct Clinical Assistant Professor at the University of Illinois College of Dentistry. He recently received his Mastership from the Academy of Laser Dentistry. Dr. Margolis is director of the Institute for Advanced Dental Education and has lectured both nationally and internationally. He has been a consultant to the ADA’s Council on Dental Practice and an ADA Seminar Series Lecturer. He has had articles published in both lay and professional publications. He is most proud of the Distinguished Member of 2005 awarded by the Illinois State Dental Society. Maria graduated from the University of Pennsylvania, School of Dental Hygiene & is the recipient of the 1999 University of Pennsylvania Dental Hygiene Alumni Achievement Award. She is also a 2003 winner of the Pfizer/ADHA Award for Excellence in Dental Hygiene. She earned her BA in Health Services Administration from Saint Mary’s College and a MS in Health Science from San Francisco State University. Maria is a graduate of the 2004-2006 fellowship of the California Health Care Foundation’s (CHCF) Health Care Leadership Program, administered by the Center for Health Professions at the University of California, San Francisco. She is a member of the American Academy of Dental Hygiene (AADH) and an Honorary Member of Sigma Phi Alpha, the National Dental Hygiene Honor Society. Maria is visiting faculty at the University of Rome for the Interdisciplinary Master’s Degree Program in Advanced Technologies in the Sciences of Oral Hygiene. Dr. Joseph DiDonato

Dr. Joseph DiDonato is a 1981 graduate of NYU and completed a GPR at the Eastman Department of Dentistry at the University of Rochester, where he continues to teach and attend. In 2001 he completed an MBA at the Rochester Institute of Technology. He is a Fellow of the Academy of General Dentistry and Past President of the New York State AGD. He has chaired numerous audit committees. Dr DiDonato now focuses his time on fraud and embezzlement with a special emphasis on the computer environment in not for profit organizations and private practices. He is the director of the Rochester Institute of Healthcare Management, which develops curriculum and provides CE for healthcare professionals. He is the developer of DDS- Dental Decision Systems, a computer simulation of dental practice he uses to teach practice management. Dr. D is an avid amateur paleontologist who has collected dinosaur fossils on four continents. His last expedition was to Beijing to see the “The Feathered Dinosaurs of the Great Wall,” where the transition from dinosaurs to birds has been discovered. His next expedition is to Egypt to search for the “Lost Dinosaurs of Egypt.”
Dr. Jacqueline Russo
Dr. Jacqueline Russo, a graduate of Baylor College of Dentistry, worked as a cosmetic reconstructive dentist for 16 years. She was a founding member of the Seattle Study Club of Brevard County, and active in of the local, state and national dental associations. She earned her BS in Nursing from Indiana University in 1978, and has been a passionate student in the early detection and prevention of disease throughout her career.
In 2001, she then began consulting in the field of ergonomic and efficient office design and is currently a principle of EnviroMed Design Group, a specialty dental office design company. Her area of expertise is in practice management and profitability and she has evaluated over hundreds of technologies, products and systems for dentists who want to increase productivity and profitability in the dental office. Dr. Aron Gonshor
Dr. Aron Gonshor is a maxillofacial surgeon, practicing in Montreal, Canada. He received his PhD in Neurophysiology in 1974, a DDS in 1975, and his maxillofacial specialty at the Montreal General Hospital - all from McGill University.
He has published extensively on implant therapy, PRP, Bone grafting and related topics, and is presently heading the Clinical Advisory Board for Ace Surgical Supply Co. He has lectured throughout North and South America, Europe and the Far East, covering topics related to implant rehabilitation, PRP therapy and wound care, as well as bone graft reconstruction.
A past president of the Canadian Association of Oral and Maxillofacial Surgeons, he is a fellow of the Royal College of Dentistry and served for 10 years as a director of the Academy of Osseo integration.
Mrs. Teresa Duncan
With over 20 years of healthcare experience, Teresa addresses topics such as Insurance Coding, Fraud & Embezzlement, and Dental Social Media, particularly the use of blogs, newsletters, and social networking to target today’s internet-savvy patient. Dr. Harvey Levy
Dr. Harvey Levy is a general dentist who has earned eight fellowships, the Mastership and the Lifelong Learning Service Recognition in the Academy of General Dentistry, and four diplomate certifications including Special Patient Care and DOCS. After graduating from Tufts Dental School in 1974, Dr. Levy completed a two-year GP residency in Rochester, NY, and taught full-time at the University of Pennsylvania Hospital GP residency. He currently heads a 4-doctor practice in Frederick, Maryland, and is chief of dental service at Frederick Memorial Hospital. With 35 years’ experience in treating special-needs patients, he is an enthusiastic and experienced lecturer at dental conferences and workshops. His work with special-needs patients earned him the 1986 AGD Humanitarian Award, the 2002 ADA Access to Care Award, and the honor of being a 2002 Winter Olympic torch runner. To date, Dr. Levy has treated over 1,000 patients in the O.R. under general anesthesia, and completed 30,000 patient visits in his office using oral sedation.

Dr. Lorin Berland
Dr. Lorin Berland is an internationally acclaimed cosmetic dentist and one of the most published authorities in the dental and general media. He is a Fellow of the AACD, the co-creator of the Lorin Library Smile Style Guide, the developer of www.denturewearers.com, the creator of `Biomimetic Same Day Inlay/Onlays` an 8 AGD Credit CD ROM and the founder of Berland Dental Arts, a multi-doctor specialty practice in the Dallas Arts District that pioneered the concept of spa dentistry. Mr. Edwin Bayo
Ed worked in various capacities for the Florida Office of the Attorney General, including Tax Litigation, Administrative Law, Cabinet Affairs and Inspector General. Ed's primary area of practice while in government involved providing advice and representation to regulatory boards under the umbrella of the Department of Health and the Department of Business and Professional Regulation. As Senior Assistant Attorney General in the Administrative Law Section, Ed served as counsel to various professional regulatory boards, including Pharmacy, Dentistry, Osteopathic Medicine, Chiropractic Medicine, Veterinary Medicine, Professional Engineers, Landscape Architecture, Clinical Social Work, and Harbor Pilots. In addition, Ed handled temporary duties or litigation for several other boards including Medicine, Psychology, Nursing, Architecture and Accountancy. In 2002, Ed served as the Attorney General Representative to the Pedigree Paper Task Force.
Ed's current practice at Metzger, Grossman, Furlow and Bayo' includes the representation of professional licensees, regulated entities and interested parties before regulatory agencies, Florida Courts and the Division of Administrative Hearings. John K. McGill is a tax attorney, CPA and MBA. He serves as CEO of The McGill & Hill Group, LLC in Charlotte, NC, and a partner with the law firm of McGill & Hassan, P.A. He is also President of John K. McGill & Company.

Dr. Tom Nabors
Dr. Nabors is Co-Founder and Senior Vice President, Chief Dental Officer, OralDNA® Labs, A Quest Diagnostics Company. He has over 40 years experience in dental and oral medicine diagnostics and disease management. He received his Doctor of Dental Surgery degree from the University Of Tennessee College Of Dentistry in 1968. After serving in the U.S. Navy from 1968-1970 and rotating through all of the dental specialties including oral surgery, periodontal surgery, endodontic therapy, prosthetic dentistry and general dentistry, Dr. Nabors practiced general dentistry for 38 years. He adopted early forms of oral fluid testing beginning in 1983. During the 26 years and following, Dr. Nabors became an advocate for a variety of diagnostic techniques including phase contrast microscopy, enzyme testing of GCF, anaerobic cultivation and DNA genetic techniques. Dr. Nabors was instrumental in establishing an ongoing working relationship with Vanderbilt University Otolaryngology department to validate saliva as a suitable molecular analyte for HPV-related oropharyngeal cancer. He has published and continues to publish numerous articles within a variety of dental publications on the subjects of molecular diagnostic techniques in oral medicine. He is a member of numerous dental associations and honorary groups including Life member status of the ADA, Associate member AAP, Tennessee State Dental Society, Fellow of the American College of Dentists, and the Pierre Fauchard Honorary Society: an international honorary group.
Dr. Nabors is a frequent, recognized lecturer for medical and dental CE courses on a variety of subjects, including the subject of molecular genetics in the field of oral medicine. Recent speaking engagements include “Preventing Heart Attacks and Stroke: Why Oral Infections Matter”, Harvard School of Dental Medicine graduate periodontal department, Baylor College of Dentistry, University of Tennessee, University of Texas graduate Periodontal department, the American Academy of Periodontology National Meeting, and numerous key study club groups throughout the United States. Dr. Mancuso graduated from the University of Toronto in 1985 and has maintained a general dental practice in Welland, Ontario ever since.
He is a fellow of the following: the Academy of Dentistry International, the Pierre Fauchard Academy, and the International Academy for Dental Facial Esthetics. In 2006 he received the award of Master in the Academy of General Dentistry. He has published articles for numerous dental journals and has given over 100 presentations to various dental organizations in Canada, the United States and internationally.
He is the current Trustee for Eastern Canada in the AGD, the Past President of the Ontario AGD and past-Mastertrack Director of the OAGD. He is on the editorial board of Spectrum-the CJDT and is also the Past-President for the Niagara Peninsula Dental Association – a component society of the ODA. Roger K. Hill is president of Roger K. Hill & Company (a member of The McGill & Hill Group), a firm specializing in dental practice transitions. Richard L. Wynn, PhD is Professor of Pharmacology at the Dental School, University of Maryland, Baltimore. He was awarded the BS, MS and PhD degrees all from the University of Maryland. He is a pharmacist in the state of Maryland holding the degree of BS in Pharmacy. He chaired the Department of Pharmacology at the University of Maryland Dental School from 1980 to 1995. Previously he chaired the Department of Oral Biology at the University of Kentucky Dental School in Lexington. He has to his credit over 300 publications including original research articles, textbooks, book chapters, and monographs. He has given over 500 continuing education seminars to dental professionals in the US, Canada, Mexico and Europe. Dr. Wynn has been a consultant to the drug industry for 25 years and his research laboratories have contributed to the development of new analgesics and anesthetics. He is a consultant to the Academy of General Dentistry, and a past consultant to the U.S. Pharmacopeia, Dental Drugs and Product Section. He is a featured columnist for the journal General Dentistry, published by the Academy of General Dentistry. Dr. Lee Ostler
Dr. Lee Ostler is a 1983 graduate of the University of Washington School Of Dentistry and has a private practice in Richland Washington. He has focused his general practice on advanced cosmetic and bite rehabilitation, comprehensive periodontal management, dental sleep medicine, facial pain and TMJ/TMD therapy.
Dr. Ostler has been instrumental in building and introducing practical solutions to help the healthcare community work together to better deliver patient care. He is co-founder of the Oral SysteMixTM healthcare concept which fully recognizes the impact of the many “mouth-body” relationships.
Dr. Ostler is co-developer of The MedDentETC (Electronic Treatment Coordinator) protocol utility software program, which builds clinical protocols, patient reports and provides automated and improved methods for inter- disciplinary communication. He is also developer of The MDReferrals program which helps health professionals build professional referral networks.
He is author and developer of several books and websites including: the EZInstantAuthor.com marketing and authoring program, OralHealthForDiabetics.com, MDReferrals.com, the Physicians Referral and Education Program (PREP), the Physicians Marketing Handbook, and 21 Principles of Smile Design - Your Guide to a More Attractive Confident Smile, among others. His articles on Professional Referral Networks have been published in Dental Economics and Inside Dentistry. He is a past clinical instructor at the University of Kentucky and Baylor University Continuums for Cosmetic Dentistry, and at the Las Vegas Institute.
With the emergence of new scientific understandings regarding the systemic inflammatory effects of periodontal disease and sleep apnea, current medical/legal standards-of-care have shifted to include multi-disciplinary management of compromising medical conditions. Dr. Ostler is a frequent lecturer to physician and dentist audiences around the country on the science of inflammation and modern oral-systemic principles which necessarily incorporate these principles of inter-disciplinary care.
Dr. Ostler is a Bale/Doneen Preceptor, a founding member of the American Academy for Oral Systemic Health, and is editor of the AAOSH Connect newsletter, and developer and webmaster of the AAOSH.org website.
Bradley Bale MD
Dr. Bradley Bale is one of the nation’s leading specialists in preventing heart attacks, stroke and diabetes. Since 2002, he’s given more than 600 speeches and presentations to medical groups in the US and abroad, including the American Heart Association, Berkeley HeartLab and European Atherosclerosis Society. His media interviews include The Huckabee Show, Fox TV, ABC TV, The Wall Street Journal and ESPN, which recently reported on how Dr. Bale screened coaches, scouts and former players from Seattle Seahawks football team for cardiovascular disease (CVD), using a state-of-the-art ultrasound test. He has also served as a medical expert on the Larry King Cardiac Foundation’s program on sudden cardiac death, honoring the late Tim Russert.
Dr. Bale is medical director of the Heart Health Program at Grace Clinic in Lubbock, Texas and cofounder of the Bale/Doneen Method international preceptorship program, an accredited continuing medical education (CME) course teaching health care providers advanced techniques to detect—and reverse—CVD before the patient suffers a heart attack or stroke. Hundreds of MDs have completed the training, some of whom subsequently opened their own prevention clinics, hailing the course “an incredible resource...the future of medicine,” “the best, by far, educational program I have ever attended,” and “the education of a lifetime...the only totally comprehensive program in the world for the prevention of this disease.”
For more than 30 years, Dr. Bale maintained one of the largest primary care practices in Spokane, Washington, where he cofounded the Heart Attack Prevention Clinic with Amy Doneen, MSN, ARNP in 2003—one of the first such facilities in the US. (It’s now called the Heart Attack & Stroke Prevention Center). Convinced that standard methods of cardiac care weren’t doing enough to identify early stages of CVD and avert recurrences in heart attack and stroke survivors, he cofounded the Bale/Doneen Method of personalized risk assessment and disease management in 2001. Of the 3,500 patients he’s treated with the method—including many physicians with CVD— only one had a heart attack, from which the patient made a full recovery. None of them suffered strokes.
A sought-after speaker and educator, Dr. Bale is co-chair of Berkeley Heartlab’s national five-hour CVD prevention course, which he will present in 18 cities from coast to coast this year. He made a similar tour last year, and in 2008, he lectured in 20 cities as co-chair of a national CME program on lipids and CVD. Dr. Bale has presented study abstracts at the 2009 American Heart Association meeting, the 2009 International Atherosclerosis Society meeting, the 2007 European Atherosclerosis Society Meeting, the 2005 American Heart Association conference on Arteriosclerosis, Thrombosis and Vascular Biology, and many others.
His research on CVD prevention has been published in such respected medical journals as the American Heart Association’s Circulation, Atherosclerosis, the official journal of the European Atherosclerosis Society, Post Graduate Medicine, Journal of National Medical Association, Journal of Clinical Lipidology, Physician’s Weekly, and Journal of Clinical Lipidology, ADVANCE for Administrators of the Laboratory and ADVANCE for Healthy Aging. He has also served as a reviewer for American Journal of Cardiology, a consultant to deCODE Genetics and Cleveland HeartLab, and is on the speakers’ bureaus for Takeda, diaDexus and Berkeley HeartLab.
Dr. Bale is a clinical assistant professor, School of Medicine, Texas Tech Health Sciences Center and an adjunct professor at Texas Tech’s School of Nursing. He received his Doctor of Medicine degree from University of Kentucky and his Bachelor of Arts in biochemistry from Lawrence University. He was board-certified in family medicine in 1978 and is a member of American Heart Association, American Stroke Association, American Diabetes Association, American Lipid Association, Council on Arteriosclerosis, Thrombosis and Vascular Biology, Society of Atherosclerosis Imaging and Prevention and other medical societies. He has participated in numerous Grand Round and CME presentations and spoken at many major medical conferences.
Amy Doneen ARNP
Amy Doneen is an international leader in preventing heart attacks, stroke, and diabetes. She has a strong focus on women’s heart health utilizing her skills to define risk assessment and treatment. Since 2002, she has given more than 600 presentations to global medical groups, including to the International Atherosclerosis Society, International Symposium on Atherosclerosis, National Lipid Association Symposium, Women’s Heart Symposium, American Heart Association, Berkeley Heart Lab, and Cleveland Heart Lab. She has been interviewed by major radio and TV news shows, including ESPN reporting the screening of coaches, scouts, and former players from the Seattle Seahawks football team for cardiovascular disease, and WBRZ in Baton Rouge. She has also been published in regional, national and Internet publications.
Doneen is owner and medical director of the Heart Attack & Stroke Prevention Center in Spokane, Washington and cofounder of the Bale/Doneen Method for heart attack, stroke, and diabetes prevention. She presents AAFP accredited continuing medical education (CME) Preceptorship courses several times a year in cities across the United States, training health care providers in such advanced CVD detection techniques as genetic testing, state-of-the-art ultrasound measurements of major arteries, high-tech heart scans, personalized treatment strategies and “fire panel” tests for insulin resistance— cornerstones of the patented Bale/Doneen Method.
Passionate about educating fellow providers about how to save lives, Doneen has been described in a magazine profile as a “risk detective” because the Bale/Doneen Method’s mission is to find the root cause for why patients’ arteries become inflamed and could ignite future heart attacks and strokes if the fire isn’t extinguished early enough. She was co-chair of a national continuing medical education (CME) course for doctors on residual lipid risk in CVD, presented in 20 cities in both 2008 and 2009 and participated in a national Grand Round CME course on metabolic syndrome.
Doneen is an adjunct professor at Texas Tech University Health Science Center School of Nursing and has delivered numerous keynote addresses on women’s heart health at major medical conferences. She did general undergraduate studies at Western Washington University and Eastern Washington University, and received her Associate Degree in Nursing (ADN) and Registered Nurse (RN) from Spokane Community College in 1996. After working as an registered nurse for several years, she returned to school and received her Bachelor of Science in Nursing (BSN), Master of Science in Nursing (MSN), and Advanced Registered Nurse Practitioner (ARNP) degrees from Gonzaga University, graduating summa cum laude in 2003.
While working on her graduate thesis, she was shocked to discover, through an extensive review of medical literature, how few studies have been done on CVD in women. She also learned that many women at risk for heart attacks and strokes are missed by the current standard of care, with devastating or even fatal consequences. During her graduate studies, she met Dr. Bradley Bale, who had revamped his family medical practice to pioneer state-of-the-art CVD prevention techniques, such as screening patients for early stage vascular disease using electron beam tomography. Doneen created a thesis project using patients in the practice, comparing the widely used Framingham Risk Score, which predicts CVD risk through such factors as age, gender, cholesterol levels, blood pressure, and smoking status, against checking patients’ arteries for calcium deposits--a better method of evaluating future risk for heart attacks and stroke. Out of this collaboration was born the Bale/Doneen Method for prevention of heart attack, stroke, and diabetes.
Doneen is an international board member and consultant to deCODE Genetics and has served on an international advisory board on the utility of carotid intra-medial thickness, an ultrasound test that screens for heart disease by measuring the amount of plaque in neck arteries. She is chair of Pacific Northwest Chapter of the Preventive Cardiovascular Nurses Association, and has served on the steering committee for “Healthy U” at Deaconess Medical Center. She is on the speakers’ bureau of Abbott Pharmaceuticals, Cleveland HeartLab, and Berkeley HeartLab.
She’s a member of the Society of Atherosclerosis, Imaging, Prevention; American Heart Association; American Stroke Association; Coalition for the Diagnosis and Treatment of Peripheral Artery Disease; and National Lipid Association, and others. Doneen has presented her ambulatory research at the International Symposium for Atherosclerosis in Rome, Italy in 2006 and in Boston, in 2009. She also presented data at the European Atherosclerotic Society in Helsinki, Finland in 2007 and at National Lipid Association Annual Symposium in New York City, NY, 2011. In 2009 and 2010, Doneen was a member of a 12 person international committee, for the definition and clinical utility of cIMT with the SAIP Society. In 2007, she was a member of a 4 person National Committee for development and allocation of CME Grand Round presentation on metabolic syndrome. Dr. Duane Keller
Dr. Duane Keller graduated from Washington University School of Dental Medicine and has maintained a successful private practice in St. Louis, MO since 1976. In addition to general dentistry, Dr. Keller works with physicians and physical therapists to provide on-surgical treatment for TMD patients in chronic pain. As the Chief Scientific Officer for Perio Protect, LLC and a former education officer for the IAO, Dr. Keller has been lecturing to national and international audiences for over twenty years. He has published more than 35 articles in professional journals. He has also patented numerous dental appliances and treatment methodologies.
In 2004, the FDA cleared his Perio Tray for the placement of solutions in periodontal pockets. The minimally invasive Perio Protect Method® combines mechanical and chemical therapies to disrupt oral biofilm growth. The objectives of the Method are to break up the matrix and change the microbiological environment of pockets to disrupt biofilm growth.

Mr. Maccario is president of Dental Management Sciences, LLC, a business management firm for dental practices. Bob earned his M. B. A. from Pepperdine University. He is a veteran of the dental field, with over 40 years of experience.
