The first part of the course will go over in detail the various aspects of data collection for a complete Periodontal examination. Specifically the following topics will be covered: appropriate documentation, examination instruments, American Academy of Periodontology Parameters of Care, sequence of data collection, Periodontal Screening & Recording, probing (health versus disease), recession, clinical attachment level, bleeding on probing, mucogingival defects & pathology, mobility, furcation involvement classifications and periodontal diagnosis. The second part of the course will evaluate the use of local delivered, antimicrobial agents as an adjunct for non-surgical periodontal therapy. Specifically, PerioChip, Atridox and Arestin will be discussed as to their indications, contraindications, clinical use and effectiveness. In addition, the systemic use of Periostat will be covered. Studies from the literature will shed interesting findings with regard to changes in probing depths, clinical attachment levels and bleeding on probing.

It has been shown that there is a great deal of subjectivity and inconsistency when diagnosing periodontal disease. This is true whether one practitioner is working patient to patient, or when multiple clinicians are viewing one patient’s data. Traditionally, there has been a ‘two-dimensional’ approach to diagnosing based on clinical conditions and patient medical history. However, in order to optimally treat disease, it is important that a ‘third dimension’ be added to the picture—that of risk. Even when one understands the concept of risk, there have always been challenges in accurately determining it, but doing so in a way that is practical in a private practice setting. This program discusses what risk is, how to determine it, how it is distinct from current disease state, and how knowing both allows for proper treatment planning and dramatically improves quality of patient care. This course begins with some comments about the current understanding of chronic inflammation related to the periodontal diseases. The concept of risk is defined and a risk assessment tool is presented. Various risk factors including why biofilm- bacterial plaque- is necessary but not sufficient to cause inflammatory periodontal disease, the transmission of pathogens, the role of calculus, tooth anatomy, restorative factors, systemic factors such as diabetes, obesity, HIV/AIDS, medications, genetics, smoking, and other factors may contribute to the progression of attachment loss are explored in detail. The course concludes with a section on the criteria needed to prove a causal relationship between periodontitis and systemic diseases such as cardiovascular disease and diabetes. The proper interpretation of odds ratios closes out the session.
